Admissions Check List

The Tri-Cities Prep Application consists of multiple documents. These documents provide us with an opportunity to see each applicant as a student, and as a member of the educational community. The following check list should help you organize your application. All forms must be submitted to the office in order for us to process your application.

□ 2012-13 Application Form (PDF)

o This form is to be completed by both the parent(s) and student. Please note that the student portion of this form is used as an evaluative tool by the Admissions Committee. All student essays must be handwritten.

□ 2012-13 Student Transfer Parental Certification Form (PDF)

o To be completed by parent.

□ 2012-13 Confidential School Report/Transcript Request Form (PDF)

o The first page of this form is to be completed by the applicant’s parent and then given to the Principal and or designee of your current school.

□ 2012-13 Confidential School Evaluation Form (PDF)

o The first page of this form is to be completed by the applicant’s parent and then given to the Principal and or designee of your current school.

□ 2012-13 Transcript Request Form

All forms are to be faxed or mailed to Tri-Cities Prep no later than Friday, February 17, 2012. All applications will be numbered by order received and once maximum acceptance has been reached a waiting list will be created.

Applications will not be processed unless ALL forms are received.
